This form is issued in the Netherlands by the NVWA and serves to support the transport of live animals that have been on the road for more than 8 hours.
The document is in use all over the EU.

Some features of this form:
a. PrintCMR contains all 5 pages of the Journey Log. To print pages 1 and 2, this has to be cut loose for use on many printers.
Since the cutting edge will not always be clean, page 2 can also be printed upside down. The bottom has a fixed distance from the input boxes, so that the print will always be properly placed.
b. All pages can be printed including the layout. This takes care of the application, where pages 1 and 2 must be submitted to the government agency without a serial number. As an extra button, a printout can be made especially for this, so that this request can be sent in 1 PDF.
c. The document is also suitable for use on iPad or iPhone.
- Signatures now to be standardized
The signatures on all forms can now be fully standardized.
From a legal point of view, this solution is not that strong, but practice shows that there is a need for it.
The signature boxes have now also been added via the Logo form behind the document.
As soon as a Sign-On-Glass signature is placed on the form itself, it will take precedence over the drawing from the Logo form.
- Weighing ticket error fixed
The weight slip contained an error in the "Made up at" field, which could not be changed under certain circumstances.
This has now been resolved.
